Visitor Security Form

CAR Governing Committee meetings will be held at the AIB Conference Center on the 7th floor at 101 Arch Street in Boston. If you will be attending a Governing Committee meeting, please complete this form in order to expedite your clearance through security in the main lobby of our building. Please note that if you are a member of the committee that is scheduled to meet, you are not required to notify CAR of your plans to attend.

CAR Advisory Committee meetings will be held virtually via Zoom video conferencing software. If you will be attending an Advisory Committee meeting, please complete all the information on the Visitor Security Form so that we may provide you with meeting access information and accurate attendance can be taken for the record. Please do not share meeting access information, as each individual is required to complete a Visitor Security Form.
